How do I get affiliates for a phobia cure that really works?

What is the best way to get affiliates for our web site?  We
have a
program, it actually curers phobias, which is on a CD, which
can be
bought for $59.95, or they can stream that online
immediately for
$49.95. We give a 60 day, one hundred percent moneyback
refund, and
only 2% ask for their money back.  This thing works great. 
How do I
find affiliates, good ones?
How much should we pay an affiliate for sending someone to
us that
buys?  What is the best, most economical software for
tracking, where
someone came from, which page they visited and who gets the
commission?
 So far, we have a conversion rate of about one out of 20.

I do affiliate marketing for a number of vendors.
What I would do is

1) Join ClickBank. You have access to thousands of
affiliates
if you put your product into CB's marketplace or you can
recruit
just a few targeted affiliates and still offer them the
security
of getting paid twice a month through CB.

2) Your 1/20 conversion rate is very good. However, wouldn't
it make sense to take the contents of the cd, put it into a
zip
file and offer it as an immediate download?

3) Did I mention join ClickBank?

I forgot one other point.

A 50/50 split is customary; 40% affiliate commission is
so-so,
depending on the price of the product and anything less than
that
is something I personally ignore unless there are other
factors
that make it an ideal product for me to represent.

I say this as an affiliate and as someone with products for
sale
as a merchant. I believe in paying affiliates well; 50% of a
bigger
pie is probably bigger than 60% of a smaller one.
