sorry to annoying you again with this topic, but, thank to
suggestions 
received today, the survey that you find at the following
link has been 
improved:

Now the number of compulsory questions is limited to nr. 1,
5, 7 and 12. 
It seems to work properly now.
At this time I am interested in public librarians' answers,

are any in the list, the questionnaire is mainly for them.
