steps to reproduce:

on zones, log in as lenya. for simplicity, add user lenya to
group reviewers.
submit any document, check "notify".
go to your inbox, read the message. a new, identical message
will appear with an
updated timestamp.

after some submitting and publishing, more and more spurious
messages crop up -
ultimately, several bogus notifications are generated for
every action done.
