i don't like this approach, because it adds an extra sitemap
match in default,
even though the problem originates in the core.
first, it's just a workaround, and second, other
publications will have to add
more boilerplate code to their sitemap.

i have created an alternative patch that tackles the issue
directly in
menu2xhtml.xsl. the problem is that i don't feel at home
with that code and the
xsl is very generic and thus hard to read. perhaps you can
get the basic idea
and verify whether the patch is correct...

i have also removed the /index.html redirects and changed
them to /, because the
sitemap will take care of redirection. that way, the
information about index
redirection is centralized (still not optimal, but better).

warning: this thing has not been tested very much - but it
works for the usecase
