The update handler could call the processor when it was time
to do the manipulation too.
Consider the case of future support for ParallelReader,
where some fields are in one sub-index and some fields are
in another sub-index. I'm not sure if our current processor
interfaces will be able to handle that scenario well (but
I'm not sure if we should worry about it too much right now
either).
There is another way to perhaps mitigate the problem of a
few frequently modified documents causing thrashing: a
document cache in the update handler.
