I have access denied using "Deny all" inside a
<Directory /> and an "Alias
/dirname/" pointing to a directory with access enabled
using a "<Directory
/dirname/" and "Allow all".

If that directory is missing people get a 403 error instead
of a 404 error.

This bug also happens in 2.2.2

Do you get the 403 error when you create the directory
"/dirname" with
sufficiant permissions as well?

You did not told us what "Order" directive you set
inside <Directory />. Try to
set "Order Deny,Allow" inside <Directory />.
Currently, I suppose you have a
configuration problem.

After all, could you please post the relevant parts of your
configuration?
