math comment moderator

There is a Velocity macro called #MathCommentFormItem() that
could be used
in your templates. 

I'm reluctant to add conditional code to all the templates
because it seems
like it'd get messy if you were doing checks for various
plugins. It might
be a good idea to add the relevant bits to the
"bootstrap" user to have the
math comment moderation enabled there by default and then
when new blogs are
created, it's available right from the get-go.
