In a list of stories, where the status is "P",
what are the determining factors that will pick whether the
"P" displayed is red (needs to be published), or
green (published version)?

According to David, the relevant fields in the story table
are:
publish_status
current_version
published_version
...for my work (manually adjusting some stories' publish
status), the "published_version" was the field I
missed to get the Status to look correct.

I still stand by my assertion that a more detailed status
(i.e. spelled out) is much more valuable than coloured
letters

Perhaps, but there's not a real-estate issue on the workflow
list.

Hah! There isn't? Seems pretty crowded to me.
Anyway, Marshall reworked it quite a bit in trunk. See if
you like it
any better there.
Best,
David
