Jim,
What is attached to the switchport? We saw this happen with
our Unity
server one time. I think we mistakenly turned on auto qos
and had trust
cisco phone on the port. The fact that it wasn't a phone
was causing
the COS not to be trusted. This in turn would remove EF
setting when
exiting the switch. The fix was to put the command mls qos
trust dscp
on the port.

It leaves it and comes into the 4500 as DCSP 0
I could see a COS trust map issue putting it to something
else.. But

Also check your trunk to the 4500 and make sure your
trusting DSCP...
