I still get this error on random users post upgrades (
server 5.1.1 and
mapi 1.2.3 )

Which one is that? The server dll? How do you go about
updating that?
And why didn't it upgrade during the upgrade?
Web interface says "MAPI Version: 1.2.3"

No. Sorry for the confusion (we've been confused here for
quite a while
ourselves till we got clarifications from the developers),
but it turned
to be the opposite: you can use the newer server-side code
with the old
client (but you'll get warnings on each Outlook startup),
but not vice
versa.

Upgrade the server first, then upgrade the clients.

That won't work. You can upgrade the client part only after
upgrading
the server (server-part).
