Is there a standard template to put into videos that advises
viewers of 
the license that the video is available under? Ideally, this
would be 
simple and as easily recognizable as the "FBI
Warning" commonly found in 
commercial works.

I've made up such an image for people to use if they desire:

I think it's pretty cool. But there is one thing that might
be improved...

As a simple pragmatic measure, wouldn't it be cheaper for
Creative
Commons if the intertitle refered viewers to the license URL
rather than
 suggesting postal mail as the first option for getting a
copy of the
license? (Postage and paper presumeably costs CC more than
bandwidth).

I don't meant that the address shouldn't appear, just that
the URL
should probably appear (and probably first or in larger
print).

Needless to say, it'd also be good to have variants for the
other licenses.

That's really cool.

It would be nice if the screen had a brief description of
the rights
instead of the address, like:

        You may distribute verbatim copies in any medium for
        non-commercial use.
        
        See http

        details.

ND makes that pretty easy; you don't have to make a point
about
attribution, since you can't change the work. However, for,
say, CC by
2.5, maybe:

        You may distribute verbatim copies, modified
versions or samples
        in any medium and for any purpose if you retain the
title,
        copyright notices, credits for the creator(s) or
organization,
        list of any changes, and notice of this license.
        
        See http://cre
ativecommons.org/license/by/2.5/ for details.
