Great -- I know people are always confronted with how to do this.
Perhaps consider adding the license URL? (

this all looks great and if it's ok with you, CC would love
to link
to it when we revamp the information sections of our site so
that
this can help people apply CC licenses to videos.
i second Fred that the text should ideally be edited to read
"To view
a copy of this license, visit [url] or send a letter
to..."
also, on the first sample video - maybe think about removing
the CC
in a circle logo that appears in the background towards the
end. it
is extremely important that there is no confusion with the
closed
captioning symbol that has been used for many years by the
deaf
community. there shouldn't be a problem because CC license
notification does not require or need the CC in a circle but
it is
important to keep this in mind...

I third the need for the license URL in preference to the
mailing
address if need be. You might want to swap in the
new/international
version of the attribution icon, which will be available in
vector form
from http://creativeco
mmons.org/presskit soon.
