Simon,

I think you have to do some tunning with the configuration:
NCHILDREN=15
MAXCHILDREN=15

each one for lmtpd, imapd and pop3d in dbmail.conf

the number os processes cannot be bigger than the number os
processes that 
MySQL accepts
MySQL by default has a limit to 100 i think, you have to see
how much you'll 
need depending on the users you have.
The "register child failed" because he's trying
to create a new process 
(connection to MySQL) and he can't 'cause on the limit.

(it seams to me that is that, i may be wrong)
