I'm trying to get everything lined up for migrating to
dbmail.

postfix has an aliases file (/etc/aliases) for alternative
deliveries -- like 
mailing lists

Is this in any way interfere with dbmail aliases or
alternative delivery like 
the 'dbmail-user -s' option?  (or -- how does this work
differently)

-s adds an alias to an existing user.

If you want to forward a message to an external program or
alias you  
can us -x with -t.

dbmail-users -x Myemailadresfoo.com -t
externalemailadres or
dbmail-users -x Myemailadresfoo.com -t
|/usr/bin/myscript

So if I want to do something like in dspam where they call
for an alias of:

spam:    "|/usr/bin/dspam --user mail --class=spam
--source=error"

I can replace with with

dbmail-users -x spamtacocat.net -t "|/usr/bin/dspam
--user mail --class=spam 
--source=error"


???

How is this not deliverable by someone on the outside?

Yes, but the dspam script should read the complete contents
of the  
message, end give propper return code.
But I strongly believe dat spam prevention should be done at
the MTA  
(lest end to reject while in the smtp session).

I'm not quite shure what you mean.
dbmail will bounce the message if the script fails.
