It's so simple, once you know it:
Stop running saslauthd, you don't need it.
Forget that /usr/lib/sasl2/smtpd.conf
Instead, edit /etc/sasl2/smtpd.conf:
pwcheck_method: auxprop
auxprop_plugin: sql
mech_list: DIGEST-MD5 CRAM-MD5 PLAIN LOGIN
sql_engine: pgsql
sql_hostnames: DBHOST.fqdn
sql_user: USER
sql_passwd: PASS
sql_database: dbmail
sql_select: SELECT passwd FROM dbmail_users WHERE userid =
'%u %r'
sql_verbose: no
Restart Postfix, use SMTP AUTH. Have fun.
