PATCH: DOM0_GETPAGEFRAMEINFO & DOM0_GETPAGEFRAMEINFO2 removed

Hi,

after greping in tools/ I found these two hypercalls are
never used by 
xen/ia64.  In order to lighten Xen, this patch removes them.

Tested by booting domU.
