You might want to try using IndexData's PHP/YAZ package to create this kind of simple interface. It's also possible to do your ISBN search using the common command language demo on their website at http://www.indexdata.com/phpyaz/demo/ccl.php View the source, copy the code to your own server (once you've installed the YAZ toolkit and configured your PHP instance --with-yaz) and you should be all set.
