I would guess XMLMind has a pretty good DocBook stylesheet

One problem with sending DocBook source without telling the
client to
process it is that the source can contain things like
<xi:include> and
processing instructions which CSS wouldn't be able to style.

Yeah, but XMLMind also extends CSS to make it good enough
for use in
the XMLEditor.

Revisiting the older CSS work with the aim of targeting a
very recent
CSS version may have better success, but will, of course,
not allow
users using IE.
