For those who want to customize a DocBook DTD, I made a
presentation at an
O'Reilly conference about best practices for doing so. I
added a link to
my slides from the Wiki page on DocBookCustomization:

Slides are not complete instructions, of course, but they
might help you
sort out where to make a certain change, especially with
regard to the
parameter entities with "class" and
"mix" in their names.
I include links to LiveDTD as well, a utility which helps
you see which
declaration for an item is the active one (live links) and
which have been
preempted by the customization.
