Writing a document I want to annotate the current point with
'FIXME' 
i.e. come back to this point and do something.

Do users simple write that, then search, or use XML which
could
collate and link the points in the document needing action?

Dave,

You might consider the <remark> element.  That's what
I use
when I want to do what you're describing.  I don't do any
special processing like you describe in your email, but it
shouldn't be too hard to do that in a customization.
