Has anyone here hired a professional indexer to work on
Docbook sources? 
If so, how did that go? Do you have any recommendations?

My book is (essentially) docbook.
It was indexed professionally, by an O'Reilly indexer
recommended by my editor.

It works. 

It is comprehensive.


Whether you agree with the resulting index is probably 
personal preference.
