Good card marked bad

Upon inserting a G8Ti into a running 6808, one of our G8Xi
cards immediately
started logging the following messages:

Now we can't get the card to come back, it says:

Any way to clear the remaps and rescan this card, I suspect
a weird software
glitch caused it to bounce like this to the point of
failure, since the card
has been running with out problems for years until we tried
adding a G8Ti in
another slot.
