Have To Keep Reinstalling Flash Plugin for OS X

Sometimes several times a day, pages start to come up
looking like
this:

You'll see that where there is normally Flash video, it
shows the
Quicktime logo with a question mark.
After I reinstall the plugin, it's fine.
Sometimes during the same day, it goes back to the question
mark.
