I'm pretty positive the empire size step was always
present.

And, wouldn't a better choice just be to set
EFT_EMPIRE_SIZE_STEP really 
large?

Yes, but the happiness behaviour has changed substantially
in some way
I do not understand. See my recent question about impossible
city
rapture.

Th intention of this is good, and it should scale with city
count, but
the effect is too harsh.

  Feature has been present forever, but I think it used to
default to 0 
(disabled).

  After checking S2_0: Yes, empire_size_inc was 0 for all
governments.
