support converting code with undefined namespaces

This is very common; see for example
packages/numerics/NumericsAdds.st. 
and packages/i18n/Sets.st ("I18N addSubspace:
#Encoders").  Most 
conversion failures are of this shape; I tested
DhbNumericalMethods.
