Started updates to theme docs

I started to apply a new organizational hierarchy to theme
docs. This  
organization is roughly based on an updated outline
available at  
http://2tbsp.com/.outgoing/ThemeDocumentationOutline.doc


The next steps are to

1. incorporate Beckett's work from http://codex.gallery2.org/
 
index.php/Gallery2:Themes_New

2. write documentation for theme.inc targeted at web
designers and  
PHP novices.
