Analytics showing incorrect referrals from AdWords - 2 new

Theoretically, 1 click = 1 visit. I've seen some
discrepancies when AdWords 
campaigns aren't tagged with Urchin/GA campaign variables,
and sometimes with 
AFC (AdWords for Content) clicks as well. The logical
question is: have you 
defined your campaign variables for your AdWords keywords?
