CR-Client: FLV fileformat changes to produce interleaved stream

Description
------------------------------------------
Normally the FLV fileformat produces two streams, one
for FLV video and the other for mp3 audio. However,
in some situations, in may be useful to pass both streams
interleaved together into the Flash player. Therefore,
this change adds an alternate mode to the FLV fileformat
for producing an interleaved stream.

If either: a) an URL option "InterleavedStream=1"
is added
to the URL; or b) a ULONG32 property
"InterleavedStream"
is set to 1 in the request headers, then the FLV fileformat
will produce a single interleaved stream. This interleaved
stream also has file offset information in each packet.
