(ISF) Online PDF form completion

Hi folks-

An organization we serve is interested in setting up a website that collects data from users, and then produces a very specific PDF form using that data. The function would be the same as what's available at <https://pptform.state.gov/DS11/MainDS11.aspx>, but a good deal simpler and shorter. Does anyone have experience setting this sort of thing up? Specifically, what might it cost to contract out the necessary programming and configuration?
