Nice job with the summary Chris. A couple of items for clarification:

S2) "Allow the crawlers to index the bad page, but set up one of several kinds of redirects to the new page."; - When you use the 301 redirect you are actually instructing the search engine to drop/remove that particular "bad" page from its index so that it is no longer present.

S3) "Allow the crawlers to index the bad page, but ask websites with links to it to change the link url to the correct page.";

- Probably goes without saying, but I would definitely recommend asking anyone with links to your old url/bad page to update the links to your current site, regardless of what approach you decide to take for addressing the issue of the cached page.
