(ISF) How small nonprofits evaluate IT training sessions

Dear ISF Colleagues,

I'm interested in hearing about ways to evaluate IT trainings that are
crafted around the needs of small nonprofits. Do you use a one-page
sheet that the trainees can fill out when the session is over?

The perfect evaluation "instrument"; is easy, fast, valid, and
reliable. It would be great to nail down at least three out of four.
