Rosie, take a look at iboardroom.com.  Haven't used yet but
saw a demo
at conference earlier this year and it seems to have most of
the
functionality you're looking for.

Some important questions:

Is these features for which you need to pay a premium? More 

generally, have you defined your requirements and further
than that  
list? Those features are available in a great many free, low
costs,  
and hosted forms. What sort of tech do you already have that
you want  
to build upon? Have you considered interoperability and exit
costs?
