documentation changes needed to blktrace

I just tried out blktrace and stumbled in a couple of
places:
- the documentation says 'Support for tracing block io
actions' is under 
'Block Devices', but it's really under 'Block Layer'
- when you run blktrace you get 'relayfs' errors.  the
documentation 
shows a mount command using ''-t relayfs" that
doesn't work, at least 
not on my system nor is there anything in .config to
configure it in.  
Rather, if you just "mount -t debugfs debugfs
/debug" and then run 
"blktrace -r /debug" it seems to work

Yeah, it has been moved to debugfs. I'll add that as well!
