Is there a way for MimeDefang to specify a multi-line
response here?

No; it doesn't support the smfi_setmlreply callback yet.

Sorry for another followup: This is unlikely to be supported
any time
soon in MIMEDefang, because the smfi_setmlreply API is not
very well
designed.  It takes a variable number of arguments
terminated with NULL.
There's no sane way to get the Perl filter to communicate
back to the
C Milter a multi-line reply.  Until/unless Sendmail
introduces
a smfi_vsetmlreply function that takes a single argument
that is
an array of strings terminated with NULL, MIMEDefang will
not
support multi-line responses.
