Well, I answered part of my own question.

In my case, my connectivity problem is due to a weak connection
between the USB cable and the phone. I get disconnected if I jostle
the phone or cable while I'm connected. I can get reconnected by
shutting down the computer, disconnecting and reconnecting the phone
from the cable, and turning the phone off and then back on.

I have found some REALLY cheap cables on Amazon.com (like $2.50), so
I may try a different cable to see if it makes a better connection.
But if anyone has other input, I'd still love to hear it!
