First, make sure that you (and other users) are confirming
both spam and ham,
and that you are running the process-quarantine.pl script
regularly.
Also, you have to keep SpamAssassin up to date, as the rules
change over time as
spammers learn to get around the rule. Also, look up the
SARE rulesets and
rules-du-jour to keep them up to date.
