Everything is innodb, I have not seen any
error messages in amavis like that, I am installing mysqlard right now.

Here is what I know,
at least once a
day, sometimes more I get a large amount of mail ‘stuck’ in the
active queue, the system keeps accepting mail into the queue but nothing ever
leaves. If I watch /var/log/mail I can see mail coming in, postfix is
still doing it’s rule checks and accepting messages / dropping some, if I
check /var/spool/amavis/amavis.log I can see that it is processing but it
stopped a few minutes before I got into the office and did not start up until I
restarted postfix?!? See this
Usually if mail is staying in the active queue (vs deferred), in my experience,
it has been caused by high system load which usually causes sql slowness and
consequently spamassassin timeouts.
What is the system load during these times? if you wait several minutes do you
see any timeouts (amavis) or deferred messages (postfix)? Do you have any sql
monitoring tools (like mysqlard)? What type of database are you using (innodb,
myisam)?
These are the areas that I would focus on, looks like you're halfway there
already.

I've also had problems with bayes db contention prior to
moving bayes
into sql. You can disable bayes (in amavis.conf and
spamassassin config
files) and see if the problem resolves itself. Usually when
this
happened mail was scanned very slowly (20-40 seconds per
message instead
of 3-4). At that rate messages came in faster than they
could be
processed and the queue would grow. However, things always
kept moving
along... just slowly.
