Well, the only way (apart from an amavisd-maia child dying) that you're
going to have a mail item rejected back to the upstream MTA like that is
if you've somehow set one of the $final_*_destiny settings to D_REJECT
instead of D_DISCARD in your amavisd.conf file. If you verify that
they're all properly set to D_DISCARD, then you'll need to look at a
more detailed log to see what's going on. It still sounds a lot like a
fatal error taking place, which should be recording something of
interest in the logs.
