This usually means that no spam-checking is being performed.
There are
numerous reasons this could be happening, but the most
common cause is
address-rewriting taking place at your upstream mail server.
This
causes amavisd-maia to see the wrong email address when it
receives the
mail, and since the address doesn't match any of the
addresses it knows
about, the mail falls through to the " ."
(system default) account,
which usually has spam-checking disabled. For a more

If you rule that issue out and the problem is still there,
let us know
and we can start exploring some of the other possible
causes. For now,
though, verify the address-rewriting issue--9 times out of
10 that's the
culprit.
