Thanks. I'm a dumb ass.
I had set up the default domain and then the domains and
everything to
scan, but not my single user (which was created before
setting the
domain policies).
It seems to be working now!

Most likely it's that your system-default policy is still in
its
default, "pass-through" mode, and your
newly-created domains and users
have inherited that policy. See

It's also possible that you have an address-rewriting issue,
with your
upstream mail server aliasing addresses such that
amavisd-maia doesn't
recognize them and lets them fall through to the
system-default policy.
