I have an implementation of the custom customer fields at
http://www.pinemporium.com/mm5/merchant
.mvc?Screen=SFNT&Store_Code=PE
It required a module to gather the variables from the built
in custom 
customer fields module.  But it works pretty slick.  I wrote
it today so 
have not thoroughly tested the code.  You can create an
account and then 
go back and edit the data in the two custom fields and also
see it 
during checkout.
