It is always a good idea to make regular backups and also
when doing 
upgrades.  Nothing should go wrong however and your settings
and users 
should be preserved.  You should backup the user database
and the 
surgemail.ini to be extra safe. If you are using nwauth (the
defauly 
user database) you can backup all the nwauth* files.

If you have edited your templates back them up also and
choose no to 
upgrading them when asked (unless you want to put your
changes into the 
newest ones).

I upgrade last week from version 3.7b6 to version 3.8f3.
Upgrade went 
smothly and without any major issues. Only thing is that I
lost a 
custome template I made way back. But problem was easily
fix. I think 
you should upgrade, lots of fixs are included in this
version.
