So far just him. I have seen this happen from time to time
but its usually
associated with a user using an incorrectly formated email
address. This
guys swears he entered it correctly and I tend to agree. It
didn't bear any
sign of what I've seen before.

So if it's just him, you can rule out server wide and domain wide settings.  I'll bet money he's got an accidental space in his address or a comma instead of a period.
