Are you setup with -all in your SPF Record?
Does the sending email address exist? (the MailFrom)
Here, we use code the servers like domain.com">11ServerAdomain.com 
First, create such accounts in Surgemail, then let the servers use them as From addresses.

I have a cgi which sends mail through my SM server. Occasionally, the
send is unsuccessful and the cgi logs:

Server returned SMTP code 550 ("Requested action not taken: mailbox
not found or no access.") while sending MAIL FROM command.

Since the mailbox is correct, I'm wondering what kinds of things will
cause this error at the MAIL FROM part of the transaction.

Yes.

I have no idea. If it doesn't, will that cause the error?

I use the email address submitted by the user as the From
address so 
that the recipient has the convenience of hitting Reply. Too
many 
errors by the recipients otherwise.
