This usually is caused by a faulty spam setting of some
kind, maybe 
dns/rbl lookups that are failing or a faulty dns or some
other test.

To find the cause send a message, then grab 'mail.log' and
find the 
'thread' in question, (the thread id is the number on the
left after the 
date) extract those lines and look for a jump in time, and
the 
surrounding log lines will indicate what it was doing.  If
you can't 
figure it out send us that log and your surgemail.ini (to 
surgemail-supportnetwinsite.com)
