I think you are correct about permission or file
locks.

I ran this:
/usr/local/surgemail/nwauth -path /mail/shared -nfs

and was able to do some lookups, so is there a setting
for NFS that I am missing in the .ini ?

Try exactly above in your surgemail.ini for the
authent_process, does that fix it ?
