No by default as far as surgemail is concerned in terms of
basic
connectivity there is no difference between internal and
external network
connections. Most importantly check that surgemail is
listening on the
network interface it should be for external network
connections (generally
this would be the same as for the internal network) and
check that you do
not have any server level firewall software (like windows
firewall) blocking
the ports in question.
Basic connectivity can be checked as described:

If all else fails try setting the surgemail service on an
alternative port
and see if that does work. Even port intercepting virus
scanners etc can
play havoc with normal tcp communication on occasion.
