Only messages sent via smtp authentication will cause
entries to be added, so check if the user
has smtp authentication or not.  Let me know if that doesn't
explain it and I'll have a look for some other cause.

Also check the users 'sending' information in their email
client (their from address) does match their account name 
correctly, possibly using an alias could mix this up.

OK, I think I figured out what is happening.  Turns out I
missed the 
fact that her email account was being forwarded to another
account in 
a different domain.  It is not clear to me yet if she ever
sent any 
email out of the account where the friends was "not
working".  I 
still don't know why the sender was getting snared by the
SPF and 
Spam.  But it is clear why she would not accumulate any
addresses in 
her friends list if she never did anything to cause them to
be there.
