Do you have ?
	g_user_send_max
defined, if so then that would be the cause, if not, then
show me the actual log entry showing
it blocking that user (this will be in msg*.rec logs I
think)

Maybe the block was released due to time and your change was
not relevant.

Well my guess is it was the user limit not the ip limit. 
But the log entries would be helpful in determining this,
the 
log should make it clear which limit was reached.   Once we
see those log entries we can then determine if it worked as
expected or not and then fix it if necessary.

The whitelist setting as above.

ChrisP:  Could Surgemail have a page that displays the Top-10 inbound senders & outbound (authenticated) senders?
There's a lot of info on the Status > Advanced page, but this too would be useful.
 
Top 10 Senders In:
# Emails - From - Reply-To - IP - Subject
 
Top 10 Senders Out:
# Emails - From - Reply-To - IP - Subject
would be nice, if you're going for glitz & glitter.
... this would display any account holder who's sending out emails with a non-hosted Reply-To address, which is useful for identifying local spammers that may try to infiltrate.  Note that if g_max_bad_to and g_max_bad_ip are set to a low value like 4, Surgemail can automatically make it hard for spams to get through the system.
