Graying out a menu item?

I've written an extension that is to be used by two
different kinds of users: 
MDs and their secretaries.  The secretaries should be able
to use the entire 
functionality, i.e. they should be able to use all menu
items.  However, for 
the MDs at least one menu item should be grayed out.

How do I gray out a menu item?

I could imagine that there are at least two ways, but I've
not been successful 
with any of them:

* Gray out the menu item using the dialog for associating
menu items with
  Basic commands

* Use a basic script that's executed each time OO is
started.
